CYO has revised and updated the
sports physical/preparticipation form. It covers many new aspects, most importantly being the COVID-19 pandemic. It is 5 pages long and is broken up into 3 main parts. They are:
Part 1 - General information of the athlete to be completed by parent - page 1 and the top half of page 2.
Part 2 - Permission, Release and Authorization to be completed by parent - the bottom half of page 2, page 3 and top half of page 4.
Part 3 - The actual sports physical to be completed by the doctor - the bottom half of page 4 and page 5.
Please use the attached
new CYO sports physical/preparticipation form for the upcoming school year. Please be complete and fill out all the requirements of the new form. Sports physicals are good for 13 months from the date of the exam.
There may be cases where a current sports physical on file may still be valid. Please note the below.
1. Current valid physical on file is on old CYO physical form - Parents only need to fill out Part 1 and Part 2 of the new form. (submit all 5 pages of new form)
2. Child had a physical exam during the 2020 summer using the old CYO physical form - Parents only need to fill out Part 1 and Part 2 of the new form. Then submit the old physical form (only page 1 necessary) along with the new form. (submit all 5 pages of new form)

P.S.: It is always a good idea that you make 2 or 3 copies of your completed sports physical/preparticipation form just to have on hand before you give it to your coach. .
