A Thanksgiving message from the director of CYO

Dear Members of the CYO Community:
With Thanksgiving fast approaching, our CYO staff wanted to take a moment to express our gratitude to the athletes, parents, coaches, commissioners, and officials who work together to serve God through the children in CYO. St. Paul beautifully described our sentiments when we think of you:
I give thanks to my God at every remembrance of you, praying always with joy in my every prayer for all of you, because of your partnership for the gospel from the first day until now. I am confident of this, that the one who began a good work in you will continue to complete it until the day of Christ Jesus.
It is right that I should think this way about all of you, because I hold you in my heart, you who are all partners with me in grace, both in my imprisonment and in the defense and confirmation of the gospel.                                                                                                                                                                                                                                                                Philippians 1: 3 – 7
 I am frequently reminded that CYO is about memory making with and for children. The seeds of love and goodness that you plant in the lives of young people have the potential to nourish them for years to come. When young people learn that they are loved and capable of loving others through playing on your team and in your program, you have given a child a foundation that can last a lifetime.
As CYO continues in our 85th year, it is humbling to remember with gratitude the women and men who laid the foundation upon which we are still building. They helped to build their own legacy in CYO and the legacy of what CYO continues to do and be for children today.

St. Ignatius Coach Chuck Kyle and St. Aloysius Coach and Athletic Director Sr. Mary Jean Raymond. They are among the many whose faith, love and service continues to give living memories to the children in CYO.
Please continue to pray, hope and work for all children in CYO. May we serve them with God’s love and mercy in our hearts as we share God’s many gifts with them. Happy Thanksgiving!

Sports/Physical Participation Form

CYO has revised and updated the sports physical/preparticipation form.  It covers many new aspects, most importantly being the COVID-19 pandemic.  It is 5 pages long and is broken up into 3 main parts.  They are:

Part 1 - General information of the athlete to be completed by parent - page 1 and the top half of page 2.  

Part 2 - Permission, Release and Authorization to be completed by parent - the bottom half of page 2, page 3 and top half of page 4.  

Part 3 - The actual sports physical to be completed by the doctor - the bottom half of page 4 and page 5.  

Please use the attached new CYO sports physical/preparticipation form for the upcoming school year.  Please be complete and fill out all the requirements of the new form.  Sports physicals are good for 13 months from the date of the exam.  


There may be cases where a current sports physical on file may still be valid.  Please note the below.   

1.  Current valid physical on file is on old CYO physical form - Parents only need to fill out Part 1 and Part 2 of the new form.  (submit all 5 pages of new form)  

2.  Child had a physical exam during the 2020 summer using the old CYO physical form - Parents only need to fill out Part 1 and Part 2 of the new form.  Then submit the old physical form (only page 1 necessary) along with the new form.  (submit all 5 pages of new form)   


Sports physical/preparticipation forms will eventually be collected by your coach when and if the season starts.  If there are any questions, please, please feel free to contact me.

P.S.:  It is always a good idea that you make 2 or 3 copies of your completed sports physical/preparticipation form just to have on hand before you give it to your coach.  .

North Olmsted parishes of St. Brendan, St. Clarence and St. Richard.
